I hereby certify that the following facts set forth in the above application, as well as those on any attachment(s) to this form are true and completed to the best of my knowledge.  I agree that any misstatement(s) or omission(s) as to material facts on this application may be considered sufficient cause for dismissal from employment.

I authorize BLK SWAN Restaurant to make an investigation that may include, but is not limited to current and past employment, academic credentials, credit history, criminal records:  I agree to hold BLK SWAN and any individual or organization providing such information harmless in connection with the collection or use of such information.

I understand and agree that such employment may be terminated at any time and without liability to me for continuation of salary, wages or employment related benefits.  Either BLK SWAN or I can end our employment relationship at any time.
